Rachel Polack was a daughter of Charles Polack and Rosetta van Lier. 22 October 1884 she married Leon Ezechiel Kok in Rotterdam, a son of Josephus Kok and Claartje Salomon van Gelder. Her husband however passed away in Rotterdam 21 October 1934 and was interred in the Jewish Cemetery Toepad in Rotterdam. Rachel Polack deceased 1 Mei 1941 and the funeral records at the Jewish cemetery in Rotterdam reveal that Rachel Polack was buried there.
In May 1941, Het Joodsche Weekblad published an obituary for Rachel Kok-Polack.
